High Concentration Of Sex Offenders at One Nursing Home

According to the Texas Public Sex Offender Registry, 23 of the 34 registered sex offenders who live in Schulenburg reside at one address: 507 West Ave. That’s the location of Paradigm at the Oak nursing home.

That concentration caused a number of locals to contact The Record seeking more information.

The Record attempted to contact the facility’s parent company, Paradigm Healthcare. We wanted to know why so many sex offenders live at the facility, and whether other residents or their legal guardians know about it. They have not responded. We also called the facility administrator, but she refused to be quoted in the paper and said a company representative would call us. We have not heard back from anyone at Paradigm Healthcare.

According to ownership information from the U.S. Centers for Medicare and Medicaid Services (CMS), Paradigm Healthcare has owned the facility since 2023. Before that, it was known as Colonial Care Center.

CMS data show that the facility is certified for 90 beds. That means more than a quarter of the residents there are registered sex offenders. Three of the 23 sex offenders listed on the state registry are classified as high risk. These offenders were not just accused, but convicted, by courts on charges ranging from sexual assault to possession of child pornography.

One of them served a prison sentence for aggravated sexual assault of a five-yearold female and a 10-year-old male. Another man living there was convicted of burglary of a habitation with intent to violate or abuse the victim sexually, and the victim in that case was a 60-year-old woman.

On the website for Paradigm at the Oak ( www.paradigmattheoak. com), the facility boasts of being “Five-Star Quality Rated” by the CMS. But the facility’s profile at Medicare.gov says it has a four-star rating for quality measures and an overall rating of just one star – “Much below average,” according to the federal government.

A major penalty in 2023 likely has something to do with that. According to Medicare. gov, a complaint inspection dated May 15, 2023, detailed a physical assault that took place inside the facility on April 24 of that year.

The report said an uninvolved resident notified the nurse’s station about the attack. When nurses arrived to the room they found that “Resident #1 was laying on (the) floor in (a) fetal position unconscious with Resident #2 hitting him in the head with his fists.” A subsequent medical examination found multiple fractures in the victim’s face, the report said, and the victim suffered a concussion.

Moreover, the report said “Resident #2 had a history of aggressive behaviors and actions towards other residents. Resident #1 has a history of wandering in the unit and has known wandering behavior. Resident #1 wandered into Resident #2’s room and staff failed to intervene.”

The federal government fined Paradigm at the Oak $133,715 over the incident.

The Texas Department of Health and Human Services (HHS) regulates nursing homes in Texas. The Record has filed an open records request with HHS for information about any state investigations into physical or sexual abuse at the facility in the last two years. We’ll report on those findings when we receive them.
